Online Registration!

Dear Directors,

This page will be your "home room" for online registration.  After you have assembled all of data from your company members, observers, etc.--using the "Registration Worksheets" provided to you earlier online-- the next step is to fill out the online forms and submit them to the Long Beach Ballet.

If your computer uses Outlook as its default email service (recommended), returning the filled-in forms is just a simple click.  If you are not able to set up Outlook to handle this for you, the registration forms can be saved in your computer and subsequently sent to the Long Beach Ballet as an attachment using whichever email service you are accustomed to.  Both of these options will be made available to you upon completion of each form.

There are five separate forms types to be completed, in this order:

1 - Company and Director Registration Form
     Your Company's travel plans and whether you wish to perform at Disneyland.
     Director registration including hotel room requests and travel plans.

2 - Festival Package Registration Form
     One form per hotel room is to be used to register all participants (except Directors) who will be residing at
     the Festival Hotel (up to four participants per room). If you save these forms to your computer for subsequent
     attachment to an email to the Long Beach Ballet, make sure you rename each separate form as you go or
     you will write over the first form you saved as you save the next form.  (If you use Outlook, this problem is
     alleviated, as each form will automatically be returned to the Long Beach Ballet upon completion.)

3 - Day Observer and Private Resident Festival Participant Form
     One form per person.

4 - Program and Technical Information form
     One form per ballet to be performed.

5 - Summary Sheet
     A summary of your entire registration to be printed and returned to the Long Beach Ballet with each
     participants' emergency release form and final payment.

Please take a few moments to familiarize yourself with the content on the forms and the manner in which they are to be filled out.  As many variables as possible have been included on the forms.  There might, however, be an exception that was pre-approved by the Long Beach Ballet that is not on the form and should be explained on the final Summary Sheet.
